1. You must fill in the first box under the blue line with all your information.
After that, because all the other boxes are required to have information in them. Put the letter *n* in all the boxes you are not using. Just fill in only the box you are wanting an answer to. We will reply using your email usually with 24 to 48 hours after getting your request for the price(s) and any down payment and shipping you may need to pay. Instructions are on our About Us tab.

2. If you are contacting us about a gold item price. Put the item number(s) of the items you want a price on. Include any ring size(s), and length(s) for any necklaces or bracelets. If you need a different ring size put that information in the box also. Put all that information in the *Your Gold Item(s) Price Check Box*. We will reply back usually with 24 to 48 hours from the day we get your request for pricing.

3. If you are wanting to use or *Gold Item Layaway Plan*. Put the item number(s) of the item(s) you want a price on. Include any ring size(s), length(s) for any necklaces or bracelets. If you need a different ring size put that information in the box also. Put that information in the *Your Layaway Item(s) Price Check Box*. After submitted. We will text you if you are approved and include the day you can dome back to our website and pay your down payment. Layaway instructions are on our *About Us* tab, under the Using Our Gold Jewelry Layaway Plan.

4. If you are contacting us about financing a Scented Items & Supply order. Put the item names and the quantity of each items you want financed in the *Financing A Scented Items & Supplies Order Box*. We will reply back with the total of your item(s) and the down payment amount and shipping cost amount you must pay to put your choices on layaway. Instructions are on our About Us tab under the Financing Scented Items & Supplies topic.

5. For a Personalized Gold item(s) price check. Put the item number(s) of the items you want a price on. Include any ring size(s), length(s) for necklaces or bracelets. If you need a different ring size put that information in the box also. Put all information you want done to your item(s) in the *Personalized Item(s) Price Check Box*. Instructions are on our About Us tab under the topic, Personalized Items.

6. If you are contacting us about Gold Cash Ordering prices. Put the item number(s), ring size(s) and any length(s) for necklaces or bracelets. If you need a different ring size put that information in the box also. Instructions are on our About Us tab under the If You Want To Cash Order Gold Jewelry topic. Put all that information in the *Cash Ordering Gold Item Price Check Box*.

8. If you are contacting us with a question. Put your question in the *Comments Box* only.

When finished filling in the appropriate box to get your request answered. Click the submit button at the bottom of this form.
